脐灸疗法对溃疡性结肠炎患者BTNL2-HLA信号通路影响机制的生物信息学研究

摘要:目的  觀察脐灸疗法治疗缓解期溃疡性结肠炎(UC)的临床疗效,及其对患者结肠黏膜内BTNL2-HLA信号通路调控情况,探讨其治疗UC可能的作用机制。方法  采用随机数字表法将70例患者分为常规组和脐灸组各35例。常规组予柳氮磺吡啶肠溶片1.5~2 g/d,口服;脐灸组在常规组基础上行脐灸疗法,每周2次。2组均连续治疗12周。观察2组临床疗效,采用生物信息学逆向挖掘与BTNL2相关的调控分子及互作网络以确定其潜在的调控通路,实时荧光定量PCR分析脐灸疗法对缓解期UC患者结肠黏膜内相关靶点的调控。结果  脐灸组完全缓解率为82.86%(29/35),常规组为51.43%(18/35),脐灸组明显优于常规组(P<0.05)。2组均可调控BTNL2-HLA信号通路关键靶点分子的表达,脐灸组对HLA-DQA1和HLA-DRA的调控效果更佳。2组对HLA-DMA、HLA-DQA2、HLA-DRB1无调控效果。结论  脐灸疗法对UC患者的治疗效果较好,BTNL2-HLA信号通路是其主要的作用通路之一。

Abstract: Objective To investigate the effects of umbilical moxibustion on the treatment of ulcerative colitis (UC) in remission stage; To analyze the regulation of BTNL2-HLA signaling pathway in the colonic mucosa of UC patients during the treatment; To speculate the mechanism of umbilical moxibustion in the treatment of UC. Methods Totally 70 patients were divided into control group and umbilical moxibustion group according to random number table method, with 35 cases in each group. The control group was given sulfasalazine enteric-coated tablets 1.5 to 2 g/d orally. In the umbilical moxibustion group, umbilical moxibustion was performed on the basis of the control group, twice a week. Both groups were treated continuously for 12 weeks. The clinical efficacy of the two groups was compared. The bioinformatics was used to reversely mine BTNL2-related regulatory molecules and interaction networks, and their potential regulatory pathways were further determined. RT-qPCR was used to analyze the regulation of umbilical moxibustion on the related targets in the colonic mucosa of the above UC patients. Results The complete remission rate was 82.86% (29/35) in the umbilical moxibustion group and 51.43% (18/35) in the control group. The umbilical moxibustion group was significantly better than the control group (P<0.05). Bothgroups could regulate the expression of key target molecules in BTNL2-HLA signaling pathway, and the umbilical moxibustion group had better control effect on HLA-DQA1 and HLA-DRA. The two groups had no regulation effect on HLA-DMA, HLA-DQA2 and HLA-DRB1. Conclusion Umbilical moxibustion has a good therapeutic effect on UC patients. BTNL2-HLA signaling pathway is one of the main pathways.
